If so, your student must have an Action Plan on file at school to give us clear and vital information on your students condition and in case they need intervention. This is very important to have on file at school; all teachers will be notified and all action plans with go with your student on field trips.
Linked above are the Action Plans for the upcoming school year and the medication authorization form. Please take these forms to your child’s doctor over the summer, so your child can be ready to roll in the new school year. Remember that each medication kept on PISD campuses must have their own med authorization form (MEDICATION FORM PISD.pdf). (2 meds, 2 forms) Please have these filled out before bringing medication to the clinic in the new school year.
** Per PISD policy, your child is NOT allowed to carry their own medication, ALL medication must come through the clinic. Students that need to self-carry inhalers or emergency or diabetic medication must have an Action Plan on file, with a signature from their doctor. This is for all PISD campuses.
